San Diego Girls Exposure Camp

Big City Hoops will be hosting a Girls Exposure Camp in the month of May. The registration form for this camp is listed below. The camp will be held at the Salt Creek Community Center in Chula Vista, CA. Camp is limited to grades 6th – 9th. Spots will be limited, register today to reserve your spot. CAMP WAIVER

Acknowledgement of Risk: I hereby acknowledge that I have voluntarily chosen to allow my child to attend Big City Hoops Summer Camp. I understand the risks involved in these activities. I recognize that each camp activity involves risk of injury to my child and I agree to accept any and all risks associated with it. By voluntarily allowing my child to participate in camp activities and with the knowledge of the risks involved, I hereby agree to accept any and all inherent risks.

Hold Harmless: In consideration of allowing my child to participate in Big City Hoops Exposure Camp and to the fullest extent permitted by law, I agree to hold harmless Big City Hoops and Salt Creek Community center/Chula Vista Park and Rec, as well as its Board of Trustee, officers, employees, and its volunteers and assigns from and against all claims arising out of or resulting from my child’s participation in camp. I hereby voluntarily hold harmless and release Big City Hoops and Salt Creek Community center/Chula Vista Park and Rec, its trustees, officers, employees, agents and volunteers from any and all claims arising out of or incident to my child’s participation in Big City Hoops Exposure Camp, which may be made on behalf of me, my child, my personal representatives and my heirs or assigns. Child Will Abide By Rules and Regulations. I have instructed my child to cooperate and comply with all reasonable directions and instructions received from camp staff. I understand that any violation of camp rules will result in consequences, and ultimately dismissal from camp. I understand that if my child is dismissed from the camp, I will not receive a refund for any unused portion of the prepaid camp fee.
